Updated Staffing Issues Reporting Tool

The Staffing Issues Reporting Tool is your secure way of reporting safety and staffing issues at your workplace.

The reporting tool has now been updated and split into three separate forms so that, dependant on the issue, members can complete whichever is relevant to them.

The three forms on our website are now:
1.    Staffing Issues Reporting Tool: Relating to issues with staffing numbers, shortages
2.    Fatigue Issues Reporting Tool: Relating to rosters, overtime, double shifts
3.    Violence Reporting Tool: Relating to violence/aggression against staff

Members are encouraged to enter as much detail as possible, as this will help us to compare your report with any agreed or mandated staffing or allow us to investigate any breach of the WHS act in relation to violence\aggression and fatigue in your area.

Once submitted these reports are forwarded directly to the PLOs allocated to your worksite. An ANMF (SA Branch) Officer may need to contact you to discuss your report in more detail.
